IE Browser: (I don't know why it says "Mozilla" as the browser, but it's from Internet Explorer)

Steps to reproduce:
1. Open Internet Explorer browser
2. Go to us.kogama.com
3. Log out of any active account on that browser
4. Click on the green Login button on the top right of the page
-> You should be directed to a login form with one field for your username, but it is unclickable. There's no field for your password, nor is there a button to login.
Note: I've also tried clearing the cookies and cache with no success.
